To help you keep life in focus, vision coverage through Vision Service Plan provides benefits for eye exams and vision correction treatment.
Vision plans
You have the option to enroll in vision coverage as a new hire, during Open Enrollment, or if you have a qualifying life event. Review your rates and enroll in UKG.
Base Plan
A lower-cost vision plan with lower allowances for vision correction materials.
Premier Plan
A vision plan with higher allowances for vision correction materials.

You may choose to see any in- or out-of-network provider you’d like, but you’ll generally pay less when you stay in network. Visit the Vision Service Plan website to find an in-network vision care provider near you.
Did You Know!
If you are enrolled in any of our medical, dental or vision plans, please note that you should never be turned away from care. If your providers office cannot verify your eligibility for some reason, please ask that they allow you to sign off on an authorization form that says you will be responsible if they bill your insurance and are in fact not eligible/covered.
If you opted for paperless, you will not receive a physical ID card. Otherwise, for Blue Shield HMO enrollees, every member will receive an ID card. For Blue Shield PPO enrollees, your ID card will say Individual/Family and 2 ID cards will be mailed. If you have Apple/Android Mobile Wallet, check to see if a Blue Shield Medical, digital ID has been auto uploaded for your convenience.
