Delta Dental PPO
Gives you the freedom to choose any licensed provider. However, you’ll save the most money when you see a provider within the network.
DeltaCare USA DHMO
Available nationwide. Works much like the medical HMO plans. You must select and use a primary care dentist (PCD) from the network to coordinate your care. There is no deductible or annual dental benefit maximum. If you use a dentist outside the network, you’re responsible for paying the full cost.

Use your dental benefits wisely
Here’s how to make the most of your dental benefits:
- Choose a network provider. Each time you need dental care, you have a choice of providers. Selecting a Delta Dental participating dentist will ensure you receive the highest benefits from your plan. To find a provider, go to Delta Dental.
- Present your plan identification. Give your provider's office the information they need to bill your insurance.
- Submit for a pre-treatment estimate. You should submit a request for a pre-treatment estimate for procedures and services such as crowns, inlays, bridges, and periodontics. For more information, call Delta Dental at 800-765-6003 or visit Delta Dental.
- Check your claim status.You can review Explanation of Benefits (EOB) statements, check if claims have been paid, and more on Delta Dental.
Coverage details
When you use in-network dentists, this is a percentage of lower, negotiated rates. If you use out-of-network dentists, this is a percentage of UCR limits, which are typically lower than negotiated rates. For all covered services with out-of-network dentists, you must also pay any amount over the UCR limit.
