Dental plans

You can enroll in the dental plan of your choice as a new hire, during Open Enrollment, or if you have a qualifying life event. Review your rates and enroll at UKG website.

Delta Dental PPO

Gives you the freedom to choose any licensed provider. However, you’ll save the most money when you see a provider within the network.

DeltaCare USA DHMO

Available nationwide. Works much like the medical HMO plans. You must select and use a primary care dentist (PCD) from the network to coordinate your care. There is no deductible or annual dental benefit maximum. If you use a dentist outside the network, you’re responsible for paying the full cost.

Here’s how to make the most of your dental benefits:

  • Choose a network provider. Each time you need dental care, you have a choice of providers. Selecting a Delta Dental participating dentist will ensure you receive the highest benefits from your plan. To find a provider, go to Delta Dental.
  • Present your plan identification. Give your provider's office the information they need to bill your insurance.
