Delta Dental PPO

Gives you the freedom to choose any licensed provider. However, you’ll save the most money when you see a provider within the network.

DeltaCare USA DHMO*

Works much like the medical HMO plans. You must select and use a primary care dentist (PCD) from the network to coordinate your care. There is no deductible or annual dental benefit maximum. If you use a dentist outside the network, you’re responsible for paying the full cost.

  • Choose a network provider. Each time you need dental care, you have a choice of providers. Selecting a Delta Dental participating dentist will ensure you receive the highest benefits from your plan. To find a provider, go to Delta Dental.
  • Present your plan identification. Give your provider's office the information they need to bill your insurance.
